The Community Care Innovation Collaborative supports the development of community strategies that connect people in need to health-related social supports. The scope of these supports can include food, housing, utilities, transportation, social connection, personal safety, personal assistance, education, legal assistance, service coordination, and other health-related social needs.

With this in mind, we are excited to introduce Making the Value Case for Health-Related Social Supports, an action learning program from the Community Care Innovation Collaborative.  The program is virtual, time-efficient, and offered at no cost to all Collaborative members.

  • The program: Making the Value Case for Health-Related Social Supports
  • Where and when: Online from June 26 through August 7
  • Who it's for: Individuals and teams from community-based organizations, health care providers, health plans, public agencies, and others providing health-related social supports of any kind.
  • Why we need it: We need to sustain and grow health-related social supports across Virginia. This requires making the value case to internal stakeholders, external partners, and funders.  By making the value case we mean showing the positive impacts of health-related social supports in ways that resonate with particular audiences including internal stakeholders, external partners, and funders.
  • What you'll learn: A six-step model for making the value case to your particular audiences. Steps include: 1) Understand your audience, 2) define your objectives, 3) create your value story, 4) select your value indicators, 5) design your communication strategy, 6) make the value case.
  • What you'll take away: A tailored strategy that you can use to make the value case for health-related social supports, plus the ongoing ability to apply the six-step model for others services and supports across your organization.
  • How we'll help you:  We'll provide guidance, tools, and technical assistance via three webinars, an online resource center, and individual assistance calls.
  • The support team: The support team will include staff from Community Health Solutions (CHS) and VAAACares.  Steve Horan from CHS will be the lead facilitator, bringing more than 25 years of experience helping nonprofits and public agencies make the value case for their programs and services.
  • The time-efficient format: We purposely designed the program in a time-efficient format for busy people. The three webinars are recommended but not mandatory, and the rest can be accomplished in just a few hours on your own time schedule over the six-week period.
  • The webinar dates:  We've scheduled three webinars for 2 pm to 3 pm on June 26, July 10, and July 24We recognize that this is a summer program and people have complex schedules. Please don't let any individual conflicts with the webinar dates deter you from joining. You can always ask another team member to attend, or catch up with a summary webinar recording.
