Our Purpose

• We are at the front end of a wave of innovation for integrating health care and social supports. 
• The innovation trend is toward community care networks in which organizations work together to connect people in need to health-related social supports. 
• The purpose of this Collaborative is to support development of community care networks that connect people in need to health-related social supports. 
• Together we can help each other understand the possibilities, and innovate in ways that work best for people in need across Virginia.
